摘要
目的分析子宫腺肌病临床诊断采取B超检查的价值。方法选取厦门市湖里区妇幼保健院2023年4月至2024年4月就诊的86例疑似子宫腺肌病患者,均在术前1个月内行B超检查。将手术病理检查作为金标准,计算B超诊断子宫腺肌病的诊断效能,通过Kappa检验分析B超诊断子宫腺肌病与手术病理结果的一致性。结果86例研究对象中,手术病理结果显示,50例子宫腺肌病(阳性),36例非子宫腺肌病(阴性);B超检查显示,47例阳性,39例阴性。B超诊断子宫腺肌病的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为88.00%(44/50)、91.67%(33/36)、89.53%(77/86)、93.62%(44/47)、84.62%(33/39)。B超诊断子宫腺肌病与手术病理结果的一致性较强(Kappa值=0.819,P<0.001)。结论子宫腺肌病临床诊断采取B超检查有良好的诊断效能。
Objective To analyze the value of B-ultrasonography in the clinical diagnosis of adenomyosis.Methods A total of 86 patients with suspected adenomyosis were selected from Huli District Maternal and Child Health Hospital in Xiamen from April 2023 to April 2024.All patients underwent B-ultrasonography within 1 month before surgery.Taking surgical pathology as the gold standard,the diagnostic efficacy of B-ultrasound in the diagnosis of adenomyosis was calculated,and the consistency of B-ultrasound in the diagnosis of adenomyosis and surgical pathology was analyzed by Kappa consistency.Results Among the 86 research subjects,the pathological results showed that 50 cases of adenomyosis(positive)and 36 cases of non-adenomyosis(negative).B-ultrasonography showed 47 positive cases and 39 negative cases.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of B-ultrasound were 88.00%(44/50),91.67%(33/36),89.53%(77/86),93.62%(44/47)and 84.62%(33/39),respectively.The results of Kappa test showed that the diagnosis of adenomyosis by B-ultrasonography was consistent with the pathological results of surgery(Kappa=0.819,P<0.001).Conclusions B-ultrasonography has good diagnostic efficacy in the clinical diagnosis of adenomyosis.
