摘要
目的探讨温针灸联合黄芪桂枝五物汤加减对气虚血瘀型下肢动脉硬化闭塞症(LEASO)的治疗效果。方法收集2022年12月至2024年2月广州市荔湾中心医院收治的86例气虚血瘀型LEASO患者的临床资料,按照治疗方案的不同将患者分为观察组(n=41,常规西医治疗的基础上加用温针灸联合黄芪桂枝五物汤加减治疗)和对照组(n=45,接受常规西医治疗)。比较两组患者的LEASO病情、西医治疗情况、踝肱指数及临床疗效。结果两组患者的LEASO病情和西医治疗情况比较,差异均无统计学意义(P﹥0.05)。出院时,两组患者的踝肱指数均高于本组入院时,且观察组患者的踝肱指数高于对照组患者,差异均有统计学意义(P﹤0.05);两组患者均未发生明显的出血;观察组患者的总有效率为92.7%(38/41),高于对照组患者的71.1%(32/45)(P=0.010)。结论对于气虚血瘀型LEASO,在西医治疗的基础上加用温针灸联合黄芪桂枝五物汤加减治疗,可显著提高疗效,安全性高,值得临床推广。
Objective To investigate the therapeutic effect of warming acupuncture and moxibustion combined with Huangqi Guizhi Wuwu decoction on lower extremity arteriosclerosis obliterans(LEASO).Method The clinical data of 86 LEASO patients with qi deficiency and blood stasis type admitted to Guangzhou Liwan Central Hospital from December 2022 to February 2024,and divide the patients into observation group(n=41,received warm acupuncture and moxibustion combined with Huangqi Guizhi Wuwu decoction on the basis of conventional western medicine)and control group(n=45,received conventional Western medicine treatment)according to different treatment plans were collected.The LEASO condition,Western medicine treatment,ankle brachial index,and clinical efficacy between two groups of patients were compared.Result There were no statistically significant differences in the LEASO condition and Western medicine treatment between the two groups of patients(P>0.05).At discharge,the ankle brachial index of both groups of patients were higher than those at admission,and the ankle brachial index of the observation group was higher than that of the control group,the differences were statistically significant(P<0.05);neither group of patients experienced significant bleeding;the total effective rate of the observation group patients was 92.7%(38/41),which was higher than the 71.1%(32/45)of the control group patients(P=0.010).Conclusion For LEASO of qi deficiency and blood stasis type,the combination of warming acupuncture and moxibustion and Huangqi Guizhi Wuwu decoction on the basis of western medicine treatment can significantly improve the efficacy,and has high safety,which is worthy of clinical promotion.
