云南某医院重症监护室患者病原菌分布及耐药性分析 被引量:2

Pathogenic Bacterial Distribution and Antimicrobial Resistance Analysis in the Intensive Care Unit Patients of a Hospital in Yunnan

摘要 目的:探讨云南某医院重症监护室(ICU)患者病原菌分布情况及屎肠球菌、金黄色葡萄球菌、肺炎克雷伯菌、鲍曼不动杆菌、铜绿假单胞菌和肠杆菌属细菌(以下简称“ESKAPE”)及大肠埃希菌的耐药性。方法:采集2020年6月至2021年6月云南某医院ICU患者临床标本,并对其进行分离培养,培养物经全自动微生物分析系统进行鉴定和药物敏感试验,采用WHONET 5.6、SPSS25.0软件对数据进行统计分析。结果:313例ICU患者的590份临床标本中共分离出病原菌675株,其中,革兰氏阴性菌480株(占71.1%),以肺炎克雷伯菌、鲍曼不动杆菌、大肠埃希菌为主;革兰氏阳性菌140株(占20.7%),以金黄色葡萄球菌为主;真菌55株(占8.2%),以白假丝酵母菌为主;共检出ESKAPE病原菌389株(占57.6%)。结论:该医院分离自ICU患者的前五位病原菌分别是肺炎克雷伯菌、鲍曼不动杆菌、大肠埃希菌、金黄色葡萄球菌和铜绿假单胞菌,均存在不同程度的耐药现象。 Objective:To explore the distribution of pathogenic bacterial and assess the antimicrobial resistance of ESKAPE pathogens(Enterococcus faecium,Staphylococcus aureus,Klebsiella pneumoniae,Acinetobacter baumanii,Pseudomonas aeruginosa,and Enterobacter spp.)and Escherichia coli in the intensive care unit(ICU)of a Yunnan hospital.Methods:Clinical specimens from ICU patients in a hospital in Yunnan from June 2020 to June 2021 were collected and isolated for culture.The cultures were identified and drug susceptibility tested by a fully automated microbial analysis system.WHONET 5.6 and SPSS 25.0 software were used for statistical analysis of data.Results:Among 590 specimens from 313 ICU patients,675 pathogenic strains were isolated.Gram-negative bacteria constituted 480 strains(71.1%),with Klebsiella pneumoniae,Acinetobacter baumannii,and Escherichia coli as predominant species.Gram-positive bacteria accounted for 140 strains(20.7%),mainly Staphylococcus aureus.Fungi comprised 55 strains(8.2%),primarily Candida albicans.ESKAPE pathogens constituted 57.6%of isolates.Conclusion:The top five pathogenic microorganisms in this ICU were Klebsiella pneumoniae,Acinetobacter baumannii,Escherichia coli,Staphylococcus aureus,and Pseudomonas aeruginosa,all exhibiting varying degrees of antimicrobial resistance.
