EB病毒感染相关儿童传染性单核细胞增多症并发肺炎的危险因素 被引量:3

摘要 目的 分析EB病毒感染相关儿童传染性单核细胞增多症(IM)并发肺炎的危险因素。方法 回顾性分析2020年7月—2022年6月在武汉儿童医院中西医结合科住院且确诊为EB病毒感染相关IM 155例患儿的临床资料,按照是否并发肺炎,分为非肺炎组109例和肺炎组46例。收集两组患儿临床资料,采用单因素分析和多因素Logistic回归分析探究EB病毒感染相关儿童IM并发肺炎的危险因素,绘制受试者工作特征(ROC)曲线评估各危险因素的预测能力。结果 155例患儿中合并肺炎46例,占29.7%。单因素分析结果显示,年龄、热程及血肺炎支原体抗体滴度差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,年龄及血肺炎支原体抗体滴度是EB病毒感染相关儿童IM并发肺炎的危险因素(P<0.05)。年龄51个月是EB病毒感染相关儿童IM并发肺炎的最佳临界值(P<0.05)。结论 EB病毒感染相关儿童传染性单核细胞增多症并发肺炎的比例较高,年龄<51个月,血肺炎支原体抗体滴度≥1:160是EB病毒感染相关儿童IM并发肺炎的危险因素,在临床诊疗中应及时完善胸部影像学检查。 OBJECTIVE To explore the risk factors for Epstein-Barr virus(EBV)infection-associated infectious mononucleosis(IM)and pneumonia in the children.METHODS The clinical data were collected from 155 children who were diagnosed with EBV infection-associated IM and were treated in department of integrated traditional Chinese medicine and western medicine of Wuhan Children's Hospital,Tongji Medical College,Huazhong Univer-sity of Science and Technology from Jul 2020 to Jun 2022.The children were divided into the pneumonia group with 46 cases and the no pneumonia group with 109 cases according to the status of complication with pneumonia.The clinical data were collected from the two groups of children.Univariate analysis and multivariate logistic re-gression analysis were performed for the risk factors for the EBV infection-associated IM and pneumonia.The pre-dictive efficiencies of the risk factors were evaluated by means of receiver operating characteristic(R()C)curve.RESULTS Among the 155 children,46 were complicated with pneumonia,accounting for 29.7%.The result of un-ivariate analysis showed that there were significant differences in the age,duration of fever and blood antibody titer of M.pneumoniae(P<0.05).The result of multivariate logistic regression analysis indicated that the age and blood antibody titer of M.pneumoniae were the risk factors for EBV infection-associated IM and pneumonia in the chil-dren(P<0.05).The 51 months of age was the optimal critical value for the EBV infection-associated IM and pneumonia.CONCLUSION There is a high proportion of the children with EBV infection-associated IM and pneu-monia.The less than 51 months of age and blood antibody titer of M.pneumoniae no less than 1∶160 are the risk factors for the EBV infection-associated IM and pneumonia.It is necessary to perform the complete chest imaging examination during the clinical diagnosis and treatment.
