摘要
目的 分析低分子肝素联合滋肾育胎丸治疗复发性流产的临床效果及其对凝血指标、不良事件的影响。方法 回顾性选择2018年10月—2021年9月泰州妇产医院收治60例不明原因复发性流产患者临床资料进行分析,按随机数表法分为两组,每组30例。对照组采用阿司匹林联合泼尼松治疗,研究组在对照组基础上应用低分子肝素联合滋肾育胎丸治疗,对两组临床效果、凝血指标及不良事件进行比较。结果 研究组治疗后凝血酶时间(TT)为(15.38±0.67)s、部分活化凝血酶时间(APTT)为(32.27±1.35)s、凝血酶原时间(PT)为(14.98±1.07)s、纤维蛋白原(FIB)为(2.78±0.56)g/L,均优于对照组,差异有统计学意义(t=13.041、12.370、10.638、3.459,P<0.05)。研究组子痫前期、前置胎盘、产后出血等并发症总发生率为16.66%,低于对照组的43.33%,差异有统计学意义(χ^(2)=5.079,P<0.05)。研究组保胎成功率为80.00%,高于对照组的53.33%,差异有统计学意义(χ^(2)=4.800,P<0.05)。两组不良事件、新生儿畸形发生率比较,差异无统计学意义(P>0.05)。结论低分子肝素联合滋肾育胎丸治疗复发性流产患者,可改善胎盘血液循环,减少并发症的发生率,提升患者的保胎成功率,有效改善妊娠结局。
Objective To analyze the clinical effect of low molecular weight heparin combined with Zishenyoutai pill in the treatment of recurrent abortion and its influence on coagulation indexes and adverse events.Methods The clinical data of 60 patients with unexplained recurrent abortion admitted to Taizhou Maternity and Gynecology Hospital from October 2018 to September 2021 were reviewed and analyzed and divided into two groups according to random number table method,with 30 patients in each group.The control group was treated with aspirin combined with prednisone,and the study group was treated with low molecular weight heparin combined with shishen-yuetong pill on the basis of the control group.The clinical effects,coagulation indexes and adverse events of the two groups were compared.Results The study group of thrombin time(TT),partially activated thrombin time(APTT),prothrombin time(PT)and fibrinogen(FIB)after treatment were(15.38±0.67)s,(32.27±1.35)s,(14.98±1.07)s,(2.78±0.56)g/L,respectively,which were better than those of the control group,the difference was statistically significant(t=13.041,12.370,10.638,3.459,P<0.05).Team of preeclampsia,placenta previa,postpartum bleeding complications such as the total incidence in the study group of 16.66%,less than 43.33%of the control group,the difference was statistically significant(χ^(2)=5.079,P<0.05).Group spuc success rate of 80.00%in the study group,higher than that of 53.33%in the control group,the difference was statistically significant(χ^(2)=4.800,P<0.05).There was no statistically significant difference in the incidence of adverse events and neonatal malformations between the two groups(P>0.05).Conclusion The combination of low molecular weight heparin and Zishen Yutai Pill can improve placental blood circulation,reduce the occurrence of complications,increase the success rate of fetal protection,and effectively improve pregnancy outcomes in patients with recurrent miscarriage.
