摘要
目的探讨名老中医治疗颈痹用药规律和特点,发现核心用药和配伍以及名老中医对痹证的治则治法与核心病因病机。方法以名中医传承平台(FangNet)为数据源。利用相关数据挖掘软件进行频次统计分析、关联规则分析、系统聚类及复杂网络分析。结果共检索到480条方剂和267味中药。药物四气以温性为主,五味以辛、甘、苦为主,主要归肝、脾经。关联规则分析发现23618项,系统聚类得出3大类。复杂网络分析得出核心药物群是川芎、地龙、葛根、天麻、甘草、当归等20味药物。结论川芎是颈痹治疗最为核心的药物,核心配伍是源自桂枝加葛根汤的主要药物组成,重用活血化瘀之品,善用风药和虫类与藤类药物。名老中医认为颈痹主要是风湿邪气为患,阳气闭阻所致。以“通”为要,采取和调营卫,祛风除湿,通利和升发阳气的基本治疗原则。从中可以归纳出以“川芎/桂枝加葛根汤—‘通’—风湿邪气”为枢轴的名老中医颈痹治疗脉络。
Objective To explore the medication rules and characteristics of distinguished traditional Chinese medicine(TCM)veterans in treatment of cervical arthralgia syndrome and find the core Chinese medicines and combinations,treatment principles and methods,and core etiology and pathogenesis of arthralgia.Methods FangNet was used to obtain the data.Frequency analysis,association rule analysis,cluster analysis,and complex network analysis were carried out using relevant data mining software.Results A total of 480 prescriptions were retrieved,involving 267 Chinese medicines,which were mainly warm in four properties and pungent,sweet and bitter in five flavors and mostly act on the liver and spleen channels.Association rule analysis found 23618 items,and 3 categories were obtained by cluster analysis.Complex network analysis revealed 20 core Chinese medicines,including Rhizoma Ligustici Chuanxiong,Lumbricus,Radix Puerariae,Rhizoma Gastrodiae,Radix Glycyrrhizae,and Radix Angelicae Sinensis.Conclusion Rhizoma Ligustici Chuanxiong is the most important medicine in treatment of cervical arthralgia syndrome,and the core combinations were derived from the main components of Guizhi plus Gegen Decoction,in which blood-activating and stasis-resolving medicines were emphasized,and wind medicines as well as insect and rattan medicines were well managed.Distinguished TCM veterans believe that cervical arthralgia syndrome is mainly caused by wind-dampness evil qi and blocked yang qi,and the fundamental treatment principles are,on the basis of“dredging”,harmonizing nutrient and defensive aspects,dispelling wind,removing dampness,and promoting and elevating yang qi.It is concluded that the clue,“Rhizoma Ligustici Chuanxiong/Guizhi plus Gegen Decoction‘dredging’wind-dampness evil qi”is pivotal for treatment of cervical arthralgia syndrome.
