摘要
目的探讨分级诊疗模式下的团队照护对社区老年人群衰老后衰弱综合征的管理效果。方法收集2019年1—12月在上海市同仁医院住院的衰老后衰弱综合征患者96例,按照随机数字表法分为对照组及观察组,每组各48例。观察组由上海市同仁医院专科-全科-社区照护团队分层联合管理,对患者进行疾病管理、用药指导、康复心理、护理等干预;对照组给予常规方案进行健康宣教、护理及治疗。2组患者分别于干预前、干预后3个月、干预后6个月进行Fried衰弱表型、日常生活能力评估量表(ADL)、跌倒评估量表(MFS)、简易健康量表(SF-36)的评估。结果观察组干预前后Fried衰弱表型评分、ADL评分、MFS评分、SF-36评分较前明显好转,差异有统计学意义(P<0.05);对照组干预前后对比,差异无统计学意义(P>0.05)。结论分级诊疗模式下的团队照护模式可以有效改善社区老年衰弱患者的衰弱表型、日常生活能力、跌倒风险以及健康水平。
Objective To explore the effect of team care on the management of post-aging frailty syndrome in community elderly population under the hierarchical diagnosis and treatment mode.Methods A total of 96 patients with post-senility syndrome hospitalized in Shanghai Tongren Hospital from January to December 2019 were randomly divided into control group and observation group,48 cases each group.The subjects were administered jointly by the neurologist,general medicine department and community family doctor in different levels.The observation group received disease management,medication guidance,psychological rehabilitation,nursing and other interventions.The control group received routine health education,nursing and treatment.Fried weakness phenotype,Ability of Daily Living Scale(ADL),Morse Fall Scale(MFS)and SF-36 were respectively evaluated before intervention,3 months after intervention and 6 months after intervention.Results Fried weakness phenotype score,ADL score,MFS score and SF-36 score of the observation group were significantly improved after intervention(P<0.05).There was no significant difference in the control group after intervention(P>0.05).Conclusions The team care model under the graded diagnosis and treatment model can effectively improve the frailty phenotype,daily living ability,fall risk and health level of elderly frailty patients in the community.
