摘要
目的探讨腹腔镜探查肠粘连松解术治疗粘连性肠梗阻的临床效果,并观察其对胃肠功能恢复及并发症的影响。方法回顾性分析2018年1月—2020年11月行手术治疗的粘连性肠梗阻108例的临床资料,根据手术方式分为研究组56例和对照组52例,研究组行腹腔镜探查肠粘连松解术,对照组行传统开腹肠粘连松解术。比较2组临床疗效、围术期情况、胃肠功能恢复及并发症发生情况。结果2组总有效率比较差异无统计学意义(P>0.05)。研究组术中出血量少于对照组,手术时间、下床活动时间、住院时间及术后肠蠕动恢复时间、肠鸣音恢复时间、排气时间、排便时间均短于对照组(P<0.01)。研究组术后并发症发生率低于对照组(P<0.01)。结论腹腔镜探查肠粘连松解术治疗粘连性肠梗阻临床效果显著,有利于患者胃肠功能恢复,且术后安全性高,并发症少。
Objective To explore clinical efficacy of laparoscopic enterolysis in treatment of patients with adhesive intestinal obstruction(AIO)and its effects on recovery of gastrointestinal function and complications.Methods Clinical data of 108 patients with AIO treated surgically between January 2018 and November 2020 was retrospectively analyzed.According to operative methods,the patients were divided into research group(n=56)and control group(n=52).Research group underwent laparoscopic enterolysis,while control group underwent traditional open surgery and enterolysis.Clinical efficacy,perioperative conditions,recovery of gastrointestinal function and complications were compared between two groups.Results There was no significant difference in the total effective rate between two groups(P>0.05).Compared with those in control group,volume of intraoperative bleeding was less,while operative duration,time of off-bed activity,length of stay,postoperative recovery time of intestinal peristalsis,recovery time of bowel sounds,exhaust time and defecation time were significantly shorter in research group(P<0.01).The incidence rate of complications in research group was significantly lower than that in control group(P<0.01).Conclusion Laparoscopic enterolysis in treatment of patients with AIO may achieve significant clinical efficacy,and it is conducive to the recovery of gastrointestinal function with good postoperative safety and few complications.
