摘要
目的:观察清肺通腑汤灌肠辅助治疗对医院获得性肺炎(HAP)肺热腑实证的影响。方法:选取120例医院获得性肺炎患者,按随机数字法分为常规组、对照组、治疗组各40例。常规组予单纯西医治疗,对照组和治疗组均在常规组基础上分别给予清肺化痰汤口服、清肺通腑汤灌肠辅助治疗。疗程10天。观察三组患者5项(咳痰、腹满便秘、发热、胸痛、口干欲饮)证候积分、肺部感染影像CT评分以及全血白细胞、中性粒细胞参数的变化。结果:三组患者5项证候积分均较治疗前改善(P﹤0.05);治疗组疗后5项证候积分均较对照组和常规组改善明显(P﹤0.05)。肺部感染影像CT评分:治疗组疗效优于对照组和常规组(P﹤0.05)。全血白细胞、中性粒细胞水平:治疗组明显优于对照组和常规组(P﹤0.05)。结论:清肺通腑汤灌肠辅助治疗HAP肺热腑实证有较好疗效,可改善患者的临床症状,减少炎症因子释放,促进肺部炎症吸收。
Objective:To observe the effect of Qingfei Tongfu decoction for adjuvant treatment by enema on hospital acquired pneumonia(HAP)with lung heat and FU-organ excess syndrome.Methods:120 cases of selected patients were randomly divided into conventional group(40 cases),control group(40 cases)and treatment group(40 cases).The conventional group was treated purely with western medicine,while the control group and the treatment group were treated respectively with oral Qingfei Huatan decoction and Qingfei Tongfu decoction for adjuvant treatment by enema on the basis of conventional group.The course of treatment was 10 days.Scores of 5 syndromes(expecting sputum,full abdomen with constipation,fever,chest pain,dry mouth with desire to drink),CT score of lung infection image and changes of levels of whole blood white blood cells and neutrophils were observed in three groups of patients.Results:1.Symptom scores:After treatment,the scores of 5 syndromes of patients in three groups were improved compared with before treatment(P﹤0.05);The scores of 5 syndromes in treatment group were significantly improved compared with control group and conventional group(P﹤0.05).2.CT score of lung infection image:The curative effect of treatment group was better than those of control group and conventional group(P<0.05).3.Levels of whole blood white blood cells and neutrophils:The treatment group was significantly better than control group and conventional group(P﹤0.05).Conclusion:The clinical effect of Qingfei Tongfu decoction for adjuvant treatment by enema on HAP with lung heat and FU-organ excess syndrome can improve the clinical symptoms of patients,reduce the release of inflammatory factors,promote the absorption of lung inflammation.
