摘要
目的研究大剂量甲氨蝶呤(MTX)治疗儿童急性淋巴细胞白血病(ALL)的疗效和毒副作用。方法选取2017年8月至2019年7月,在郑州大学第一附属医院治疗的72例ALL患儿,将其分为两组。对照组36例,使用小剂量MTX治疗;观察组36例,使用大剂量MTX治疗。比较两组患儿治疗的毒副作用情况和疗效。结果观察组患儿的总有效率为94.44%(34/36),对照组为77.78%(28/36),差异有统计学意义(P<0.05)。观察组患儿的皮肤黏膜损害、神经损害、心电图异常、心肌酶谱异常评分均明显高于对照组(P<0.05)。结论对儿童ALL采取大剂量MTX治疗可取得显著疗效,但毒副作用明显,应加强血药浓度监测,提高用药的个体化水平。
Objective To study the efficacy and toxicity of high dose methotrexate(MTX) in the treatment of childhood acute lymphoblastic leukemia(ALL). Methods Seventy-two children with ALL who were treated in the First Affiliated Hospital of Zhengzhou University from August 2008 to July 2019 were divided into two groups. Thirty-six patients in the control group were treated with low-dose MTX;36 patients in the observation group were treated with high-dose MTX.The toxicity and side effects of the two groups were compared. Results The total effective rate was 94.44%(34/36) in the observation group and 77.78%(28/36) in the control group, the difference was significant(P<0.05). The scores of skin and mucosa damage, nerve damage, electrocardiogram abnormality and myocardial enzymogram abnormality in the observation group were significantly higher than those in the control group(P<0.05).Conclusions High-dose MTX treatment for ALL in children can achieve remarkable curative effect, but the toxicity and side effects are obvious. It is necessary to strengthen the monitoring of blood drug concentration and improve the individualized level of drug use.
