3D打印技术结合经心尖导管主动脉瓣植入15例 被引量:13

Fifteen cases of aortic regurgitation treated by interventional therapy in combination with 3-dimentional printing technology

摘要 目的探讨经心尖导管主动脉瓣植入术(TA-TAVI)结合3D打印技术治疗主动脉瓣关闭不全(AI)的可行性及有效性。方法回顾性研究2018年4月至6月于西京医院接续就诊的AI患者15例,并成功施行TA-TAVI。通过术前采集AI患者CT影像数据,数据导入Mimics软件将患者CT数据进行重建,结合3D打印技术制作重建AI瓣膜模型及相关解剖结构模型,即术前影像学评估。数字减影血管造影(DSA)下行TA-TAVI,术中经食道超声监测并评价手术情况,术后1、3、6个月及每年进行随访观察,结合影像学及3D打印技术进行术后评估。结果15例AI患者成功施行TA-TAVI手术,3D打印模型能够准确辅助TA-TAVI术前评估,术中手术情况和术后3D打印模型与实际手术情况相符。结论3D打印技术结合TATAVI治疗AI具有可行性与有效性。 Objective To investigate the feasibility and effectiveness of transapical transcatheter aortic valve implantation(TA-TAVI)in combination with 3-dimentional(3 D)printing in the treatment of aortic valve insufficiency.Methods A retrospective study was conducted on 15 patients with AI who were admitted to Xijing Hospital from April to June of 2018.TA-TAVI was successfully performed.CT image data of those patients were collected before operation.Mimics software was used to reconstruct the patients’CT data.Combined with 3 D printing technology,the reconstructed aortic insufficiency valve models and related anatomical structure models were made as preoperative imaging evaluation.TA-TAVI was performed by DSA angiography.Transesophageal ultrasonography was used to monitor and evaluate the operation.Follow-up observation was carried out 1 month,3 months,6 months and every year after operation.Results TA-TAVI was successfully performed in 15 patients with AI.The 3 D printing models could accurately assist the preoperative evaluation of TA-TAVI.The intraoperative and postoperative 3 D printing models were consistent with the actual operation.Conclusion 3 D printing combined with TA-TAVI is feasible and effective in the treatment of AI.
