摘要
目的分析血液灌流对维持性血液透析患者肾性骨病相关因素(c反应蛋白、β_2微球蛋白、甲状旁腺激素、血钙、血磷)的影响。方法选取该院2013年9月—2016年3月在该院治疗的维持性血液透析肾性骨病患者117例展开研究,按照随机数表法,随机分为对照组和观察组。对照组58例,采用血液透析措施对患者进行治疗;观察组59例,则采用血液透析联合血液灌流治疗。治疗前和治疗3个月后,检测分析两组患者的两组患者的临床疗效和PTH(甲状旁腺激素),c反应蛋白(hs-CRP)、β_2-MG(β_2微球蛋白)、血钙(Ca)、血磷(P)等指标变化情况,并对疗效进行分析。结果观察组患者的治疗总有效率(86.44%)高于对照组(75.86%)。将两组数据进行比较,差异有统计学意义(P﹤0.05)。治疗前,两组患者的hs-CRP、β_2-MG、Ca^(2+)、P^+等指标差异无统计学意义(P﹥0.05);治疗3个月后,对照组患者hs-CRP(7.81±2.71)mmol/L、β_2-MG(38.26±7.94)mg/L、Ca2+(2.03±0.34)mmol/L、P+(2.29±0.27)mmol/L等指标无明显变化(P﹥0.05),观察组患者hs-CRP(5.11±1.39)mmol/L、β_2-MG(22.35±6.01)mg/L、P^+(1.71±0.29)mmol/L等指标明显下降,Ca^(2+)(3.16±0.39)mmol/L明显上升(P﹤0.05)。结论对维持性血液透析肾性骨病患者使用血液透析联合血液灌流的治疗方法,能有效的改善患者的炎症状况和钙磷代谢紊乱,值得临床上推广及应用。
Objective To analyze the effect of hemoperfusion on the related factors of renal osteodystrophy(C reactive protein,β2-microglobulin,parathyroid hormone,blood calcium)of patients with maintenance dialysis.Methods 117 cases of patients with maintenance dialysis treated in our hospital from September 2013 to March 2016 were selected and randomly divided into two groups,the control group with 58 cases were treated with hemodialysis measures,the observation group with 59 cases were treated with hemoperfusion and hemodialysis measures,and the changes of clinical curative effect,PTH,C reactive protein(hs-CRP),β2-MG(β2-microglobulin),blood calcium(Ca)and blood phosphorus(P)of the two groups were tested and analyzed and the curative effect was analyzed.Results The total treatment effective rate in the observation group was higher than that in the control group,(86.44%vs 75.86%),and the differences between the two groups were statistically significant(P<0.05),before treatment,there were no obvious differences in the hs-CRP,β2-MG,Ca2+,P+between the two groups(P>0.05)after 3-month treatment,there were no obvious changes in the hs-CRP(7.81±2.71)mmol/L,β2-MG(38.26±7.94)mg/L,Ca2+(2.03±0.34)mmol/L,P+(2.29±0.27)mmol/L in the control group(P>0.05),in the observation group,the hs-CRP(5.11±1.39)mmol/L,β2-MG(22.35±6.01)mg/L,P+(1.71±0.29)mmol/L obviously decreased and the Ca2+(3.16±0.39)mmol/L obviously increased(P<0.05).Conclusion The hemodialysis and hemoperfusion in treatment of renal osteodystrophy patients with maintenance dialysis can effectively improve the inflammation status and metabolic disturbances of calcium phosphorus,which is worth clinical promotion and application.
