摘要
目的探讨不同剂量缬沙坦联合氨氯地平治疗老年高血压的应用效果。方法在本院2015年3月至2018年3月收治的老年高血压患者中选择162例作为研究对象,随机分为观察组和对照组,各81例,均采用缬沙坦联合氨氯地平治疗,其中对照组为小剂量缬沙坦(80 mg/次),观察组为大剂量缬沙坦(160 mg/次),观察两组治疗效果。结果观察组的治疗总有效率明显高于对照组(P<0.05);治疗后,两组SBP和DBP均较治疗前明显降低(P<0.05);治疗后,两组患者的血肌酐、尿素氮、24 h尿蛋白水平、内生肌酐清除率、24 h DBPV和24 h SBPV均明显改善,且观察组优于对照组(P<0.05)。结论采用缬沙坦联合氨氯地平治疗老年高血压患者可获得良好的疗效,其中采用大剂量缬沙坦治疗可改善患者肾功能,有利于进一步降低和稳定血压水平,并减少血压变异度,用药安全性较高,值得推广。
Objective To investigate the application effect of different doses of valsartan combined with amlodipine in the treatment of elderly hypertension. Methods A total of 162 elderly patients with hypertension admitted in our hospital from March 2015 to March 2018 were enrolled as the study objects and randomly divided into observation group and control group, with 81 cases in each group. They were given valsartan combined with amlodipine. The control group was given a small dose of valsartan（80 mg/time）, and the observation group was given a large dose of valsartan（160 mg/time）.The therapeutic effects of the two groups were observed. Results The total effective rate of treatment in the observation group was significantly higher than that in the control group（P〈0.05）. After treatment, the SBP and DBP of the two groups were significantly lower than those before treatment（P〈0.05）. After treatment, the levels of serum creatinine, urea nitrogen,24 h urinary protein, endogenous creatinine clearance rate, 24 h DBPV and 24 h SBPV of the two groups significantly improved, and those in the observation group were superior to the control group（P〈0.05）. Conclusion The combination of valsartan and amlodipine in the treatment of elderly hypertensive patients can achieve good efficacies. The use of high-dose valsartan can improve renal function, further reduce and stabilize blood pressure, and reduce blood pressure variability. The safety of medication is high and it is worth promoting.
