摘要
目的:评价老年患者重度磨耗伴牙列缺损行垫式可摘义齿咬合重建的疗效。方法:选取30例重度磨耗伴牙列缺损老年患者,行垫式可摘义齿咬合重建,对修复前、过渡义齿修复1个月后、永久性义齿修复3个月后的咀嚼效率、咬合力、咬合平衡性、关节前、上、后间隙进行比较并统计分析。结果:过渡义齿修复1个月后、永久义齿修复3个月后的咀嚼效率均明显优于修复前,差异有统计学意义(P<0.05),但过渡义齿与永久义齿两者间咀嚼效率无统计学差异(P>0.05)。永久义齿修复3个月后的总咬合力、咬合平衡性均优于修复前及过渡义齿修复1个月后,三者之间存在统计学差异(P<0.05)。永久性义齿修复3个月后,颞下颌关节前、上、后间隙与修复前相比差异有显著意义(P<0.01),与过渡义齿修复1个月后相比,差异无统计学意义(P>0.05)。结论:通过对患者行垫式可摘义齿咬合重建,患者的咀嚼效率、咬合力、咬合平衡性及颞下颌关节紊乱症状均能得到改善,咬合重建对于重度磨耗伴有牙列缺损的患者不失为一种有效的修复手段。
Objective： To evaluate the effect of the occlusal reconstruction of dentition defect with severe dental attrition by removable partial denture occlusal pad for elderly patients. Methods： Thirty elderly patients with severe attrition and dentition defect were selected for occlusal reconstruction with removable denture. The mastication efficiency, bite force,occlusal balance, anterior, upper and posterior space were compared and statistically analyzed before, after 1 month of transition denture and 3 months after permanent denture restoration. Results： The masticatory efficiency of transitional restoration one month later and definitive restoration three months later were both higher than the masticatory efficiency before the treatment（P〈0.05）, but there was no difference between transitional restoration and definitive restoration（P〉0.05）.The occlusal force and the balance of occlusal force of definitive restoration were better than that of transitional restoration and pre-treatment, and there was significant difference in three different period（P〈0.05）. The joint space of TMJ of definitive restoration was better than that of pre-treatment, and there was no difference between definitive restoration and transitional restoration in the joint space of TMJ. Conclusion： The mastication efficiency, bite force, occlusal balance and temporomandibular joint disorder were improved by the reconstruction of removable denture. Occlusal reconstruction is an effective method for the repair of severe attrition with dentition defect.
