摘要
目的：探讨早期应用静注丙种免疫球蛋白治疗重症或重症倾向手足口病的临床疗效。方法将80例重症或重症倾向手足口病患儿按随机数字表法分为治疗组和对照组，对照组给予常规治疗，治疗组在常规治疗基础上加用静注丙种免疫球蛋白治疗。结果治疗组总有效率（95．0％）明显高于对照组（77.5％）（χ2＝5．165，P ＜0．05），治疗组体温正常、皮疹消退及其它不良症状消失和血象复常时间分别为（2.1±177;0．9）d、（1．9±177;0．8）d、（4．1±177;1．3）d、（2．6±177;0．8）d，均相应短于对照组的（3．0±177;1．1）d、（2．8±177;1．1）d、（6．2±177;2．1）d、（4．8±177;1．5）d，差异均有统计学意义（t ＝3．934、3．723、5．378、7．357，均 P ＜0．05）。结论有重症倾向或重症手足口病早期应用静注丙种免疫球蛋白能减轻症状，快速控制病情，改善预后，安全有效。
Objective To study the efficacy of gamma globulin in the early treatment of severe or almost severe hand -foot -mouth disease.Methods 80 children with severe or almost severe hand -foot -mouth disease were randomly divided into treatment group and control group by the random number table.The treatment group received the treatment of early gamma globulin,whereas patients in the control group received conventional therapy.The effective rate of the treatment group was higher than that of the control group（95.0%vs 77.5%,χ2 =5.165,P ＆lt;0.05）.The recovery time of body temperature and hemogram,rash and other adverse symptoms disap-peared in the treatment group were significantly shorter than those of the control group[（2.1 ±177;0.9）d vs （3.0 ±177; 1.1）d,（1.9 ±177;0.8）d vs （2.8 ±177;1.1）d,（4.1 ±177;1.3）d vs （6.2 ±177;2.1）d,（2.6 ±177;0.8）d vs （4.8 ±177;1.5）d,t =3.934,3.723,5.378,7.357,all P ＆lt;0.05].Conclusion Early application of gamma globulin is effective and safe in treating severe hand -foot -mouth disease,which can relieve symptoms,control the disease quickly and improve prognosis.
