中国中小学生营养状况评价盘信效度评价 被引量:6

Reliability and validity of nutrition evaluation turnplate for Chinese primary and middle school students

摘要 目的研究"中国中小学生营养状况评价盘"(简称评价盘)的信度和效度,为学生科学进行营养状况自评、早期预防、控制营养不良和肥胖提供参考。方法选取北京市昌平区1所小学和1所完全中学的1 037名学生。根据中国肥胖问题工作组(WGOC)制定的"中国学龄儿童青少年超重、肥胖筛查BMI分类标准"和卫生行业"学龄儿童青少年营养不良筛查标准",将营养状况分为消瘦、正常、超重、肥胖。以此为金标准,分析儿童自行使用评价盘进行营养状况判定的灵敏度、特异度、符合率以及Kappa值。结果不同学段、营养状况的儿童用评价盘法和计算法得到的BMI值差异均无统计学意义(P值均〉0.05)。中学生用评价盘判定营养状况的灵敏度(75.00%-95.60%)、特异度(91.67%-99.16%)、符合率(90.85%)和Kappa值(0.83)均高于小学生(68.42%-90.07%,85.56%-96.39%,82.52%,0.70)。所有调查儿童使用评价盘和计算法进行营养状况评价的结果一致性较好(Kappa=0.77,P〈0.01)。结论采用中国中小学生营养状况评价盘不仅可以方便、快速地计算儿童的BMI值,而且能够比较准确地判定儿童青少年营养状况。 Objective To analyze the reliability and validity of the Nutrition Evaluation Turnplate for Chinese Primary and Secondary School Students. Methods Subjects were 1 037 students from one primary school and one secondary school in Changping Distriet, Beijing. The individual nutrition status (lean, normal-weight, overweight, or obesity), which was defined according to "Reference Norms for Screening Overweight and Obesity in Chinese Children and Adolescents" developed by Working Group Obesity in China(WGOC ) and "Reference Norms for Screening malnutrition in Chinese Children and Adolescents" of Health Industry, was regarded as the result of gold standard. Sensitivity, specificity, agreement rate and Kappa value were analyzed after all subjeets used the evaluation turnplate to judge nutrition status by themselves. Results There was no statistical differences between BMI values calculated by the students themselves and BMI values calculated by SPSS 17.0 among boys and girls, primary and secondary school students and students with different nutrition status (P〉0.05). Secondary school students had higher sensitivity, specificity, agreement rate, Kappa value than primary school students. Consistency of these two nutrition evaluation methods was high among all subjeets( Kappa= 0.77, P〈0.01 ). Conclusion The Nutrition Evaluation Tumplate for Chinese Primary and Secondary School Students could be used not only to calculate BMI conveniently and quickly, but also to judge nutrition status accurately among children and adolescents.
