RW垫对颞下颌关节紊乱病关节盘和髁突位置的影响 被引量:8

摘要 目的研究RW牙合垫治疗颞下颌关节紊乱病(temporomandibular joint disorders,TMD)前后关节盘和髁突的位置改变,探讨RW牙合垫的治疗机制和疗效。方法双侧关节盘前移位的TMD患者20例,分别在闭口位、最大开口位行磁共振成像扫描,测量双侧关节的盘突角度、关节盘和髁突位置的变化。结果治疗后在闭口位上盘突角度均减小,关节盘x轴坐标值均增大,髁突x、y轴坐标值均减小,差异均有统计学意义(P<0.05);最大开口位上的盘突角度、关节盘位置、髁突位置治疗前后差异均无统计学意义(P>0.05)。结论 RW牙合垫可使髁突向前、下方移动,关节盘向后回复。 Objective To investigate the change of joint disk and condyle after RW-splint treatment in patients with temporomandibular disorder( TMD). Methods Twenty patients with anterior disc displacement of the TMJ were selected to receive RW-splint treatment. All joints were scanned oblique-sagittally by MRI before and after splint treatment in both positions of closed-mouth position and maximum opening-mouth position. Measurements of both sides were made to evaluate the change. Results After treatment,for both sides of TMJ,in the closed-mouth position,the angle of disk-condyle was reduced,disk coordinate values of x were increased,condyle coordinate values of x and y were reduced. The results have significant differences( P〈0. 05). In maximum opening-mouth position,the change of the 3 aspects have no significant differences( P〉0. 05). Conclusion RW-splint made the condyle move anteriorferiorly and the disk moved posterior.
