摘要
目的探讨择期甲状腺手术患者围手术期预防性应用抗菌药物的相关情况,并分析其合理性。方法采取回顾性调查分析方法,选取医院2009年1月一2011年12月甲状腺手术患者390例,依据《抗菌药物临床应用指导原则》等分析围术期患者预防使用抗菌药物的合理性。结果手术切口甲级愈合388例、乙级愈合2例,390例甲状腺手术患者围术期应用抗菌药物共160例;围术期不合理无指征用药、选药不当、术后持续用药≥48h、联合用药、给药次数不当、剂量过大,不合理率分别为29.38%、38.75%、23.75%、15.00%、14.38%、7.50%。结论甲状腺手术患者围手术期预防性应用抗菌药物可以预防术后手术部位感染,但需要严格选择抗菌药物,注意用药时间及剂量,及时停用预防性用药,避免过度依赖抗菌药物而忽视其他预防感染的相关措施。
OBJECTIVE To investigate the status of prophylactic use of antibiotics during the perioperative period of elective thyroid surgery and analyze the rationality. METHODS By means of retrospective analysis, 390 cases of patients who underwent the thyroid surgery in the hospital from Jan 2009 to Dec,2011 were enrolled in the study, then the rationality of clinical application of antimicrobial agents during the perioperative period was analyzed in accordance with the guidelines for the clinical use of antibiotics. RESULTS The healing of surgical incision in 388 cases were grade A, two cases were grade B. Of totally 390 cases of patients undergoing the thyroid surgery, 160 cases have used antibiotics during the perioperative period. The medication without indications, improper choice of drugs, duration of postoperative drug therapy no less than 48 hours, combination use of antibiotics, improper dosing frequency, and over-dosage were the aspects of unreasonable use of antibiotics during the perioperative peri-od, accounting for 29.38%, 38.75%, 23.75%, 15.00%, 14. 38%, and 7. 50%, respectively. CONCLUSION For the patients undergoing thyroid surgery, the postoperative antibiotic prophylaxis can prevent the surgical site infections, but strict selection of antibiotics, administration time and dosage, and timely discontinuation of preven-tive medication are required for the avoidance of excessive dependence of antibacterial drugs and neglecting other infection prevention measures.
