摘要
目的探讨COP方案初始治疗低Karnofsky评分非霍奇金淋巴瘤(non-Hodgkin’s lymphoma,NHL)的效果与安全性。方法选择2008年1月—2012年12月收治的低Karnofsky评分NHL患者25例,随机分成治疗组13例和对照组12例,治疗组患者静脉化疗方案:环磷酰胺环磷酰胺600 mg/m2静脉注射,第1天;长春新碱1.4 mg/m2静脉注射,第1天;泼尼松60 mg/m2口服第1天至第5天;对照组:环磷酰胺750 mg/m2静脉注射,第1天;阿霉素50 mg/m2静脉注射,第1天;长春新碱1.4 mg/m2静脉注射,第1天,泼尼松60 mg/m2口服第1天至第5天,21d为1个疗程。观察治疗1个疗程后两组效果及Karnofsky变化情况。计数资料采用χ2检验,P<0.05为差异有统计学意义。结果治疗1个疗程后治疗组总有效率84.6%,11例Karnofsky评分>60分,对照组总有效率91.6%,2例Karnofsky评分>60分,两组总有效率比较差异无统计学意义(P>0.05),患者Karnofsky评分改善差异有统计学意义(P<0.05)。骨髓毒性治疗组发生2例占15.4%,对照组10例占83.3%,两组比较差异有统计学意义(P<0.05)。结论 COP方案治疗低Karnofsky评分NHL患者可改善Karnofsky评分,安全性好,可以在临床中应用。
Objective To observe the efficacy and safety of COP regimen in the initial treatment of non - Hodgkin＇ s lymphoma （NHL） with low Karnofsky scores. Methods From January 2008 to December 2012, twenty - five NHL pa-tients with low Karnofsky scores were randomly divided into treatment group and control group. For treatment group, CTX 600 mg/m^2 and VCR 1.4 mg/m^2 were administered intravenously on dl, Prednisone 60 mg/m^2 for oral administration on dl - dS. For control group, CTX 750 mg/m^2, ADM 50 mg/m^2 and VCR 1.4 mg/m^2 were administered intravenously on dl ; Prednisone 60 mg/m^2 for oral administration on dl - dS. One cycle lasted for 21 days. After one cycle,both groups were observed for the efficacy and Karnofsky changes. Chi - square test was used for count data, and the result of P 〈 0.05 was considered to be statistically significant. Results After one cycle, of 13 cases in treatment group, overall response rate was 84.6% ,11 patients＇ Karnofsky scores were over 60 ;while in control group, overall response rate was 91.6% ,2 patients＇ Karnofsky scores were over 60. There was significant difference in both OR and improvement of Karnofsky score between the two groups （P 〈 0.05）. Conclusions COP regimen can improve Karnofsky scores in NHL patients with low Karnofsky scores, which is safe and can be used in clinical practice.
