摘要
目的:了解浙江省首批试点基层医疗卫生机构基本药物制度实施现状和所取得的阶段性成果,针对现存主要问题提出对策和建议。方法:采用简单随机抽样方法从浙江省30个试点县(市、区)抽取60个基层医疗卫生机构进行调查,应用卫生部编制的《国家基本药物制度监测评价指标(试行)》对基层医疗卫生机构进行相关评价。结果:从试点地区人均年收入分组情况来看,实施基本药物制度后,群众用药负担明显减轻;基层医疗卫生机构业务收入下降,其中药品收入下降尤为显著;卫生服务利用量骤增等。建议:细化基本药物品种设置标准,探索有效的激励机制,加强宣传与监督,引导医患合理用药。
Objective To study the status and effect of implementing basic pharmaceuticals system in grass-roots medical institutions in pilot areas of Zhejiang Province,and put forward countermeasures and suggestions on it.Methods 60 cases of grass-roots medical institutions were randomly sampled from 30 cases of pilot areas of Zhejiang Province and surveyed with questionnaire.And they were evaluated with indexes from national system for basic pharmaceuticals monitoring and evaluation by the Ministry of Health.Results After the implementation of basic pharmaceuticals system,burden of medicines on the people has been significantly decreased.Income of medical services in grass-roots medical institutions has declined,especially that from drugs.And the amount of utilization of health care services has significantly increased.Conclusion The standard to assort basic pharmaceuticals should be refined.Effective incentive mechanism needs be explored.And publicity and surveillance on it should be strengthened to guide rational use of drugs.
