摘要
目的探索简易、快速的人曼氏裂头蚴病血清学免疫诊断方法。方法将曼氏裂头蚴可溶性抗原点样于硝酸纤维素膜上,以胶体金-SPA为检测标记物,采用垂直流渗滤装置检测病人血清中曼氏裂头蚴特异性IgG抗体。结果曼氏裂头蚴病病人血清阳性检出率为92.0%(23/25),健康人群血清假阳性率为2.0%(2/100);与囊虫病、肺吸虫病、血吸虫病、华支睾吸虫病、旋毛虫病病人血清的交叉反应率分别为40.0%(16/40)、52.0%(13/25)、13.3%(4/30)、8.0%(2/25)、6.7%(2/30),未发现与广州管圆线虫病、结核病病人血清有交叉反应;与ELISA平行检测74人份血清,两种方法检测结果的一致性有极显著性意义(К=0.871,P<0.001)。结论金标渗滤法检测曼氏裂头蚴病病人血清中特异性IgG,不仅简易、快速而且检出率高,但与肺吸虫病、囊虫病病人血清存在较高的交叉反应,需根据病人饮食史等流行病学资料进行鉴别诊断。
In this study,a simple and rapid immunoassay was established to detect the specific IgG antibodies in serum of patients with infection of Sparganum mansoni ,in which the soluble extract of plerocercoid used as antigen was dotted on nitrocellulose and protein A conjugated with colloidal gold was employed as the probe,and a filtration device with vertical flow was applied to detect the specific IgG antibodies. By using this new immunoassay,it was found that 920% (23/25)of patients with sparganosis showed positive results on the detection of serum IgG, and 20% (2/100)of healthy individuals had false positive resuits. However,high and partially high percentages of the cross reactions could be demonstrated in sera of patients with paragonimiasis or cysticercosis and in those of patients with clonorchiasis or schistosomiasis, respectively. None of patients with angiostrrangyliasis or tuberculosis showed false positive reaction. In addition,significant correlation existed in the parallel detection of this method of assay with ELISA on 74 serum specimens(К= 0. 871. P〈0. 001). It is apparent that the dot immune gold filtration assay(DIGFA)used for the detection of specific antibody IgG in sera of patients infection of Sparaganum mansoni appears to be simple,rapid and sensitive with high percentages of positive results,but it also shows higher percentages of cross reaction with sera of patients with paragonimiasis and cysticercosis. Therefore, it would be necessary to undergo differential diagnosis upon the epidemiological information,such as patient custom and behavior patterns.
