摘要
目的:评价99mTc甲氧基异丁基异腈(MIBI)双时相显像法对甲状旁腺功能亢进(甲旁亢)的诊断价值。方法:21例临床诊断甲状旁腺功能亢进患者,静注注入99mTcMIBI740MBq(20mCi),分别于10~20min(早期显像)、2h(延迟显像)采集颈部至上胸部的前位平面像。结果:5例原发性甲旁亢患者均表现为孤立的放射性浓聚灶;16例继发性甲旁亢中,2例显示为单个放射性浓聚灶,10例显示为2个放射性浓聚灶,2例显示为3个放射性浓聚灶;另有2例显示有异位甲状旁腺病灶。10例经手术证实甲旁亢患者中,术前99mTcMIBI甲状旁腺显像和超声诊断检查,对原发性甲旁亢患者定位诊断阳性率均为100%,对继发性甲旁亢患者定位诊断阳性率分别为55%和70%。结论:99mTcMIBI双时相法显像在甲旁亢患者中有较高的定位诊断价值。
Objective:To evaluate the diagnostic value of ^99mTc MIBI double phase parathyroid scintigraphy in the hyperparathyroidism patients.Methods:^99mTc MIBI double phase parathyroid scintigraphy was performed in 21 patients.Planar imaging of the neck and thorax is recorded starting 10~20 min and then 2h after the intravenous injection of ^99mTc MIBI 740MBq(20mCi).Results:Five patients with primary hyperparathyroidism had solitary lesion; and 16 patients with secondary hyperparathyroidism:pmong them two patients had solitary lesion,ten patients had two lesions,two patients had three lesions and the other two patients had ectopic parathyroid glands.Preoperative uhrasonography and ^99mTc MIBI imaging were 100% conforming to the location in the primary hyperparathyroidism,but 70% and 55% respectively in the secondary hyperparathyroidism, verified by intraoperative findings in ten patients.Conelusion:^99mTc MIBI double phase parathyroid scintigraphy is an useful imaging method for the localization diagnosis of the hyperparathyroidism.
