摘要
目的 分析发育性髋关节脱位不同治疗方式的医疗费用差异及其影响因素.方法 采用回顾性研究方法分析2015年1月1日-2017年12月31日新疆某三甲医院治疗有效的356例发育性髋关节脱位病例采用不同治疗方式的住院费用差异与影响因素.结果 女性患者占85.67%,15岁以下患者占60.11%,有医疗保险患者占67.70%.不同治疗方式住院费用的差异具有统计学意义.闭合复位术、截骨矫形术、人工髋关节置换术的患者平均住院天数分别为4.65天、5.69天、10.25天;患者次均费用分别为9583.63元、21951.00元、60590.26元.材料费和药费是患者住院总费用的重要组成部分.结论 年龄和住院天数是影响发育性髋关节脱位医疗费用的主要因素.通过避免过度医疗与规范医师行为可以有效控制材料费、特殊材料费、药费占比,从而降低发育性髋关节患者的医疗费用.
Objective To analyze the differences in medical costs and their influencing factors in different ways of treating developmental dislocation of the hip.Methods A retrospective study was conducted to analyze the difference in hospitalization costs and the influencing factors of 356 effective cases treated in a top three hospital in Xinjiang from 2015 to 2017.Results Sexual patients accounted for 85.67%,patients under 15 years old accounted for 60.11%,and patients with medical insurance accounted for 67.70%.The difference in hospitalization costs between different treatment methods was statistically significant.The average hospitalization days of patients with closed reduction,osteotomy or artificial hip replacement were 4.65 days,5.69 days,and 10.25 days,respectively.The average cost of patients was 9583.63 yuan,21,951.00 yuan,and 60,590.26 yuan.Material and medication costs are an important part of the total cost of hospitalization.Conclusions Age and length of hospital stay are the main factors affecting the cost of developmental dislocation of the hip.By avoiding excessive medical care and regulating doctor behavior,patients could effectively control material costs,special material costs,and drug costs,thereby reducing the medical costs of developmental hip patients.
