摘要
目的 探讨醋酸亮丙瑞林缓释微球联合保守性腹腔镜手术治疗重度卵巢内膜异位症的疗效及对性激素的影响.方法 选取2013年5月至2016年3月重度子宫内膜异位症患者124例,按治疗方法不同将所有患者分为对照组(n=56例)与观察组(n=68例).对照组给予保守型腹腔镜手术治疗,观察组患者在对照组患者基础上加用醋酸亮丙瑞林缓释微球治疗,观察两组患者治疗前后性激素水平、临床疗效、复发情况、治疗前后NRS评分和Kupperman评分和不良反应.结果 观察组患者治疗后卵泡生成素、黄体生成素和雌二醇水平明显低于对照组,差异有统计学意义(P<0.05).两组患者治疗后卵泡生成素、黄体生成素和雌二醇水平均明显低于治疗前,差异均有统计学意义(P<0.05).对照组总有效率83.93%;观察组总有效率97.06%.两组患者治疗总有效率比较差异有统计学意义(χ2=3.463,P<0.05).随访1年,观察组复发率5.88%,对照组复发率17.86%,两组患者复发率比较差异有统计学意义(χ2=4.354,P<0.05).观察组患者治疗后NRS评分低于对照组,Kupperman评分高于对照组,差异均有统计学意义(P<0.05).对照组不良反应发生率33.93%,观察组14.71%,两组患者不良反应发生率比较差异有统计学意义(P<0.05).结论 保守腹腔镜手术联合醋酸亮丙瑞林缓释微球治疗重度卵巢内膜异位症患者可调节性激素水平,改善患者症状体征,降低复发率,有一定的不良反应.
Objective To investigate the efficacy of leuprolide acetate sustained-release microspheres combined with conservative laparoscopic surgery in the treatment of severe ovarian endometriosis and its effect on sex hormones.Methods 124 patients with severe endometriosis from May 2013 to March 2016 were selected and divided into control group(n=56 cases)and observation group(n=68 cases)according to different treatment methods.The control group was treated with conservative laparoscopic surgery,while the observation group was treated with leuprorelin acetate sustained-release microspheres 3.75 on the basis of the control group.Sex hormone levels,clinical efficacy,recurrence,NRS scores,Kupperman scores and adverse reactions were observed before and after treatment.Results The levels of follicle hormone,luteinizing hormone and estradiol in the observation group were significantly lower than those in the control group(P<0.05).The levels of follicle hormone,luteinizing hormone and estradiol in the two groups after treatment were significantly lower than those before treatment(P<0.05).The total effective rate was 83.93%in the control group and 97.06%in the observation group.There was significant difference in the total effective rate between the two groups(χ2=3.463,P<0.05).The recurrence rate of the observation group was 5.88%and that of the control group was 17.86%.There was a significant difference in the recurrence rate between the two groups(χ2=4.354,P<0.05).After treatment,the NRS score of the observation group was lower than that of the control group,and the Kupperman score was higher than that of the control group,with statistical significance(P<0.05).The incidence of adverse reactions was 33.93%in the control group and 14.71%in the observation group.There was a significant difference between the two groups(P<0.05).Conclusion Conservative laparoscopic surgery combined with leuprorelin acetate sustained-release microspheres in the treatment of severe ovarian endometriosis can adjust the level of sex hormones,improve the symptoms and signs of patients,reduce the recurrence rate,and have certain adverse reactions.
