摘要
目的研究基于"扶正祛邪"法的针灸疗法干预带状疱疹后遗神经痛的效果.方法采用抽样便利法选取2019年12月—2020年12月就诊于上海市宝山区吴淞街道社区卫生服务中心的带状疱疹后遗症神经痛患者60例为研究对象,按照数表法将其随机分为观察组和对照组,每组30例.对照组采用常规西药治疗,观察组采用基于"扶正祛邪"法的针灸疗法治疗比较两组患者干预前后的疼痛视觉模拟评分(VAS)、疼痛改善时间、疼痛持续时间、间隔时间、生活质量及随访3、6个月的神经痛复发率.结果干预后,观察组的VAS评分为(1.65±1.03)分,低于对照组的(2.98±1.64)分,差异有统计学意义(P<0.05).干预后,观察组疼痛改善时间、疼痛持续时间、间隔时间均短于对照组,差异有统计学意义(P<0.05).干预后,观察组的躯体功能、心理功能、社会功能及总体生活质量评分均高于对照组,差异有统计学意义(P<0.05).观察组随访3、6个月的后遗症神经痛复发率分别为0.00%、3.33%,均低于对照组,差异有统计学意义(P<0.05).结论将基于"扶正祛邪"法的针灸疗法应用于带状疱疹后遗神经痛干预中的效果显著,可减轻患者疼痛,改善生活质量,操作性强、易于为患者接受,适用于基层医院,值得推广.
Objective To study the effect of acupuncture and moxibustion therapy based on“Fuzheng Quxie”on neuralgia of herpes zoster sequela.Methods 60 patients with postherpetic neuralgia after herpes zoster who were treated in the community health service center of Wusong Street,Baoshan District,Shanghai from December 2019 to December 2020 were selected by sampling convenience method as the research objects.They were randomly divided into an observation group and a control group according to the number table method,with 30 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with acupuncture intervention based on"Fuzheng Quxie"method.The visual analog scale(VAS)score of pain,pain improvement time,pain duration,interval time,quality of life,and the recurrence rate of neuralgia after 3 and 6 months follow-up were compared between the two groups before and after intervention.Results After intervention,the VAS score of the observation group was(1.65±1.03)points,lower than(2.98±1.64)points of the control group,the difference was statistically significant(P<0.05).After intervention,the improvement time,duration and interval time of pain in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After intervention,the scores of physical function,psychological function,social function and overall quality of life in the observation group were higher than those in the control group,and the differences were statistical ly significant(P<0.05).The recurrence rates of sequelae neuralgia in the observation group were 0.00%and 3.33%after 3 and 6 months follow-up,respectively,which were lower than those in the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Acupuncture and moxibustion therapy based on the method of"Fuzheng Quxie"in the intervention of postherpetic neuralgia has a significant effect,can relieve the pain of patients,improve the quality of life,strong operability,easy to be accepted by patients,suitable for primary hospitals,worth promoting.
