摘要
目的探讨InterTan髓内钉与股骨近端防旋髓内钉(PFNA)内固定治疗骨质疏松性股骨粗隆间骨折(IFF)的疗效.方法选取2020年3月—2022年1月该院收治的100例骨质疏松性IFF患者,按随机数字表法分为两组,各50例.对照组采用PFNA内固定治疗,观察组采用InterTan髓内钉内固定治疗,观察至术后3个月.对比两组手术相关指标、髋关节功能恢复情况、生活质量、并发症.结果观察组手术时间长于对照组,术中出血量多于对照组,组间差异有统计学意义(P<0.05).观察组髋关节功能恢复优良率为94.00%,较对照组的78.00%高,差异有统计学意义(P<0.05).术后3个月,观察组的生活质量综合评定问卷中心理、躯体、社会功能及物质生活状态评分均较对照组高,组间差异有统计学意义(P<0.05).两组并发症发生率比较,差异无统计学意义(P>0.05).结论与PFNA内固定治疗相比,InterTan髓内钉内固定手术时间较长,出血量较多,但能够促进骨质疏松性IFF患者髋关节功能恢复,改善生活质量,且无严重并发症,安全性高.
Objective To investigate the efficacy of InterTan intramedullary nail and proximal femoral nail antirotation(PFNA)in the treatment of osteoporotic intertrochanteric femoral fractures(IFF).Methods A total of 100 patients with osteoporotic IFF admitted to our hospital from March 2020 to January 2022 were selected and divided into two groups according to the random number table method,with 50 cases in each group.The control group was treated with PFNA internal fixation,and the observation group was treated with InterTan intramedullary nail internal fixation,and the observation was performed until 3 months after operation.The operation-related indicators,hip function recovery,quality of life,and complications were compared between the two groups.Results The operation time of the observation group was longer than that of the control group,and the intraoperative blood loss was more than that of the control group,and the differences were statistically significant(P<0.05).The excellent and good rate of hip function recovery in the observation group was 94.00%,which was higher than that of 78.00%in the control group,and the difference was statistically significant(P<0.05).3 months after operation,the scores of psychological,physical,social function and physical life status in the generic quality of life inventory-74 in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Compared with PFNA internal fixation treatment,InterTan intramedullary nail internal fixation has longer operation time and more bleeding,but it can promote the recovery of hip function and improve the quality of life in patients with osteoporotic IFF without serious complications and with high safety.
