摘要
目的探讨流程化沟通模式联合快速康复外科在腹腔镜胆囊切除围手术期患者中的应用效果。方法选取我院2021年2月—2023年2月收治的86例腹腔镜胆囊切除手术患者为研究对象,采取随机数字表法将其分为对照组与观察组,各43例。对照组采取快速康复外科护理,观察组采取流程化沟通模式联合快速康复外科护理。观察并对比两组患者术后恢复情况,手术前后炎症反应情况、睡眠质量、疼痛评分及并发症发生情况。结果观察组患者首次排气时间、首次排便时间、引流管拔出时间及术后住院天数均短于对照组,组间差异有统计学意义(P<0.05)。术后3 d,两组患者C反应蛋白、白细胞介素-6、肿瘤坏死因子-α水平均高于术前,但观察组低于对照组,差异有统计学意义(P<0.05)。术后3 d,两组疼痛视觉模拟评分、匹兹堡睡眠质量指数量表评分均高于术前,但观察组低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为4.65%,低于对照组的18.60%,差异有统计学意义(P<0.05)。结论流程化沟通模式联合快速康复外科可促进腹腔镜胆囊切除患者术后康复,缩短住院时间,减轻术后炎症因子水平上高情况,降低术后疼痛程度,改善睡眠质量,辅助降低术后并发症发生率。
Objective To explore the application effect of process communication mode combined with rapid rehabilitation surgery in perioperative patients undergoing laparoscopic cholecystectomy.Methods 86 patients with laparoscopic cholecystectomy treated in our hospital from February 2021 to February 2023 were selected as the study objects,and were divided into a control group and an observation group by random number table method,with 43 cases in each group.The control group took rapid rehabilitation surgery nursing,and the observation group took process communication mode combined with rapid rehabilitation surgery nursing.The postoperative recovery,inflammatory response,sleep quality,pain score and complications were observed and compared between the two groups.Results The first exhaust time,first defecation time,drainage tube extraction time and postoperative hospitalization days in observation group were shorter than those in control group,and the differences between groups were statistically significant(P<0.05).3 days after surgery,the levels of C-reactive protein,interleukin-6 and tumor necrosis factor-α in two groups were higher than before operation,but the observation group was lower than the control group,the difference was statistically significant(P<0.05).3 days after surgery,the pain Visual Analogue Score and Pittsburgh Sleep Quality Index score of the two groups were higher than before surgery,but the observation group was lower than the control group,the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 4.65%,lower than 18.60%in the control group,and the difference was statistically significant(P<0.05).Conclusion The combination of procedural communication mode and rapid rehabilitation surgery can promote postoperative recovery of laparoscopic cholecystectomy patients,shorten hospital stay,alleviate high levels of postoperative inflammatory factors,reduce postoperative pain,improve sleep quality,and assist in reducing the incidence of postoperative complications.
