临床医学硕士专业学位研究生医患沟通的认知及培养需求研究 被引量:13

摘要 目的探讨全日制临床医学硕士专业学位研究生(简称专硕)医患沟通的认知以及培养需求,并分析其影响因素。方法对浙江大学临床医学专硕进行问卷调查,运用主体框架分析法,进行归纳总结。结果不同年级专硕对于医患沟通能力自评得分方面有统计学意义(F=3.887,P<0.05);73.4%专硕认为医患沟通很重要,医患沟通要素中"适当作自我介绍""让病人讲述对其健康问题和/或疾病发展过程的看法""系统询问影响疾病的社会、心理、情感因素""给病人说话的时间和机会/无尴尬停顿"和"避免诱导性提问/命令式提问"得分排在最后5位。目前专硕医患沟通为选修课程,更倾向于角色扮演(59.4%)和视频教学(56.3%)的授课方式。患者对医生的信任和尊重以及医生工作量大是影响医患关系的主要因素。结论专硕在医患沟通能力重要性的认知与实践中的落实上存在差距,多元化的教学方法结合临床实践重点环节医患沟通培养,可切实提高专硕的医患沟通能力。 Objective To explore the cognition and training requirements of doctor-patient communication of postgraduates of clinical medicine(full-time)and analyze their influencing factors.Methods A questionnaire survey was conducted among postgraduate students(full-time)of Zhejiang University.And we used the main body frame analysis method to conclude and summarize.Results Statistical significance was found in selfevaluation of doctor-patient communication ability(F=3.887,P<0.05).73.4%of postgraduates(full-time)believed that doctor-patient communication was very important."Introduction to the patient","let the patient talk about their views on their health problems and/or the development process of the disease","systematic inquiring about the social,psychological,and emotional factors affecting the disease","giving the patient time to speak/no embarrassing pause"and"avoid leading questions/command questions"were the five lowest scores.At present,postgraduate doctor-patient communication was an optional course,role-playing(59.4%)and video teaching(56.3%)were the two favorite ways of students.Trust in doctors and the heavy workload of doctors were the main factors affecting the doctor-patient relationship.Conclusion There is a gap between postgraduates’recognition of the importance of doctor-patient communication skills and their implementation in practice.Diversified teaching methods combined with the cultivation in key links of clinical practice can effectively improve the doctor-patient communication ability of postgraduate students.
