摘要
目的探讨刺络拔罐疗法联合普瑞巴林治疗带状疱疹后遗神经痛(PHN)的临床效果。方法选取2021年1月—2023年12月我院收治的72例PHN患者为研究对象,按随机数字表法将其分为对照组和观察组,各36例。对照组采用普瑞巴林治疗,观察组采用刺络拔罐疗法联合普瑞巴林治疗,两组均持续治疗14 d。比较两组的临床疗效、疼痛程度、睡眠质量、炎症因子水平、疼痛介质水平及不良反应发生情况。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组视觉模拟评分、匹兹堡睡眠质量指数评分均低于对照组,白细胞介素-1、肿瘤坏死因子-α、白细胞介素-6水平均低于对照组,P物质、前列腺素E_(2)、单核细胞趋化蛋白-1水平均低于对照组,组间差异有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论刺络拔罐疗法联合普瑞巴林可提高PHN的治疗效果,加快局部炎症消退,减少致痛因子释放,并可降低疼痛程度,降低疾病对患者睡眠质量的影响,且未出现严重不良反应。
Objective Exploring the clinical effect of acupuncture and cupping therapy combined with pregabalin in the treatment of postherpetic neuralgia(PHN).Methods A total of 72 PHN patients admitted to our hospital from January 2021 to December 2023 were selected as the research subjects,divided into a control group and an observation group using a random number table method,with 36 cases in each group.The control group was treated with pregabalin,while the observation group was treated with acupuncture and cupping therapy combined with pregabalin,both groups continued to receive treatment for 14 days.Compare the clinical efficacy,pain degree,sleep quality,inflammatory cytokine levels,pain mediator levels,and adverse reactions between two groups.Results The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the pain Visual Analogue Scale and Pittsburgh Sleep Quality Index scores of the observation group were lower than those of the control group.The levels of interleukin-1,tumor necrosis factor-α,and interleukin-6 were all lower than those of the control group.The levels of substance P,prostaglandin E_(2),and monocyte chemotactic protein-1 were also lower than those of the control group,the differences between groups were statistically significant(P<0.05).Comparison of incidence rates of adverse reactions between two groups,the difference was not statistically significant(P>0.05).Conclusion The acupuncture and cupping therapy combined with pregabalin can improve the therapeutic effect of PHN,accelerate the resolution of local inflammation,reduce the release of pain inducing factors,and lower the degree of pain.It can also reduce the impact of the disease on the patient's sleep quality,and there are no serious adverse reactions.
