摘要
目的:探讨不同麻醉方法在无痛分娩中的应用效果差异。方法:选取2017年3月至2018年3月我院产科拟实施无痛分娩的产妇120例,采用随机数字表法分为A组和B组各60例,A组采用硬膜外和蛛网膜下腔联合麻醉(腰硬联合麻醉)、B组采用单纯硬膜外麻醉;对比两组不同时间点的血流动力学指标、总产程、宫口全开时间、新生儿Apgar评分、产后出血量。结果:在T0时刻A组和B组患者的血压、心率值差异均无统计学意义(P>0.05);在T1、T2时刻,A组的收缩压显著高于B组(P<0.05),A组的心率低于B组(P<0.05);A组和B组患者的总产程、第一产程时间差异均无统计学意义(P>0.05);A组的产后出血量低于B组(P<0.05);A组和B组的1 min、5min Apgar评分比较,差异不具有统计学意义(P>0.05)。结论:腰硬联合麻醉在无痛分娩中较单纯硬膜外麻醉对产妇血流动力学影响更小、产后出血量更低、对总产程影响不明显。
Objective:To compare the effect of different anesthesia methods in painless delivery.Methods:A total of 120 cases of maternal women who had painless delivery in our hospital from March 2017 to March 2018 were selected.They were divided into group A with combined spinal and epidural anesthesia and group B with simple epidural anesthesia,by random number table method.The hemodynamic parameters,total labor time,neonatal Apgar scores,and postpartum hemorrhage were compared between the two groups at different time points.Results:There were no significant differences in SBP,DBP,and HR between group A and group B at TO(P>0.05).At T1 and T2,SBP was significantly higher in group A(P<0.05),while the HR of group A was slower than in group B(P<0.05).There was no significant difference in total labor and first labor stage between group A and group B(P>0.05).Postpartum hemorrhage were less in group A versus group B(P<0.05).No difference in Apgar scores of 1 min and 5 min was found between group A and group B(P>0.05).Conclusion:Compared with simple epidural anesthesia,combined spinal and epidural anesthesia has no impact on total labor time,with less side effects on hemodynamics and postpartum hemorrhage.
