摘要
目的探讨冬季高山滑雪运动膝关节损伤机制以及MRI表现。方法搜集35例高山滑雪运动者膝关节损伤患者,共36例膝关节损伤,作为实验组;随机选取31例普通外伤膝关节患者,共31例膝关节损伤,作为对照组。采用3.0 T MRI和相控阵线圈进行膝关节扫描。由两名放射科主治医师评估膝关节骨质、软骨、半月板、韧带、肌腱、髌下脂肪垫及关节积液等病变。结果实验组多结构联合损伤36例(100%);对照组多结构联合损伤26例(83.87%)。实验组的骨挫伤/骨折、软骨损伤、半月板损伤、韧带和肌腱损伤、髌下脂肪垫损伤的发病率明显高于对照组,其差异有统计学意义(P<0.05),但实验组关节腔积液的发病率与对照组比较无统计学差异(P>0.05)。实验组的关节软骨、半月板、韧带和肌腱损伤程度高于对照组(P<0.05)。实验组内、外侧半月板比较无统计学差异(t=-0.0447,P=0.698)。实验组表现多个解剖骨挫伤/骨折,对照组表现为单个解剖部位骨挫伤/骨折。实验组关节软骨≥Ⅱ级损伤比对照组更为常见,同时伴有其他结构联合损伤,而对照组软骨损伤仅伴有软骨下骨质水肿。实验组半月板主要表现为Ⅱ/Ⅲ级损伤,对照组表现为Ⅰ/Ⅱ级损伤。实验组多表现为多条韧带联合损伤,以Ⅱ级损伤居多;对照组以单条韧带损伤为主,以Ⅰ级损伤表现居多。实验组肌腱损伤常表现Ⅱ级损伤,对照组肌腱损伤常表现Ⅰ级损伤。结论滑雪运动膝关节损伤为骨、软骨、半月板、韧带及肌腱的联合损伤,正确认识滑雪运动膝关节损伤机制和MRI表现,对早期诊断、膝关节功能恢复有重要意义。
Objective To investigate the mechanism and MRI findings of knee joint injury in alpine skiing in winter.Methods A total of 36 cases of knee joint injury were collected from 35 alpine skiers.A total of 31 patients with knee injuries were randomly selected as the control group.Knee scan was performed with 3.0 T MRI and phased-front loop.Bone marrow,cartilage,meniscus,ligaments,tendons,subpatellar fat pad and joint effusion were evaluated by 2 radiologists.Results 36(100%)cases of multi-structure combined injury in the experimental group.In the control group,26 patients(83.87%)suffered multiple structural injuries.The incidence of bone contusion,articular cartilage injury,meniscus injury,ligament and tendon injury in the experimental group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05),but the incidence of articular cavity effusion in the experimental group was not statistically different from that in the control group(P>0.05).The injury degree of articular cartilage,meniscus,ligament and tendon in the experimental group was higher than that in the control group(P<0.05).There was no statistical difference between the two groups(t=-0.0447,P=0.698).The experimental group presented multiple anatomical bone contusions/fractures,while the control group presented single anatomical bone contusions/fractures.The articular cartilage injury of grade≥2 in the experimental group was more common than that in the control group,accompanied by combined injury of other structures,while the cartilage injury in the control group was only accompanied by subchondral bone edema.the meniscus of experimental group main show is gradeⅡ/Ⅲ,control group performance for gradeⅠ/Ⅱ.Experimental group more manifested multiple joint ligament injury,in the majority with gradeⅡ;The control group is given priority to with a single ligament damage,in the majority with gradeⅠ.Tendon injuries often performanceⅡgrade in experimental group,tendon injuries oftenⅠgrade in control group.Conclusion The injury of knee joint in skiing sports is the joint injury of bone marrow,cartilage,meniscus,ligament and tendon.The correct understanding of the knee mechanism and MRI manifestations of skiing is of great significance for early diagnosis and recovery of knee function.
