摘要
糖尿病周围神经病变是糖尿病最常见的慢性并发症之一,临床主要表现为对称性肢体末端感觉和运动异常,如灼热样疼痛和/或针刺样疼痛、袜套样分布的蚁走感等,西医治疗从降低血糖、营养神经、改善微循环及对症镇痛等方面入手,轻症可以有所缓解,对于中重度发病患者效果不佳。冯建华教授认为本病的基本病机为脾气亏虚、痰瘀互结,病理过程为毒损络脉,治疗应当益气通阳养血以治本,活血化痰通络以治标,其治疗经验包括明确辨证、标本同治,临证常用经方,药物加减应用及剂量等3个方面。冯建华教授治疗时首抓疾病的标本缓急,考虑疾病发生发展的整体病机,辨证论治,屡收效验。
Diabetic peripheral neuropathy is one of the most common chronic complications of diabetes.The clinical manifestations mainly include symmetrical limb end sensory and motor abnormalities,such as burning pain and/or needling pain,sock-like ant walking sensation,etc.Western medicine treatment starts with lowering blood glucose,nourishing nerves,improving microcirculation and symptomatic analgesia.Mild diseases can be relieved,but the effect is not good for moderate and severe patients.According to Professor FENG Jianhua,the basic pathogenesis of this disease is deficiency of spleen Qi,mutual consolidation of phlegm and blood stasis,and toxin damaging collaterals.The treatment should benefit Qi,clear Yang and nourish blood to treat the root cause,and activate blood,dissolve phlegm and clear collaterals to treat the symptoms.The treatment experience includes three aspects,namely clear syndrome differentiation,common prescriptions for clinical symptoms,addition and reduction of drug application and dosage.In the treatment,Professor FENG Jianhua first grasped the urgency of the disease specimen,considered the overall pathogenesis of the occurrence and development of the disease,and treated the patients according to the syndrome differentiation and received the good efficacy repeatedly.
