摘要
目的:探讨自制复方大黄白及喷雾外治法防治鼻咽癌放射治疗所致急性放射性皮炎的临床疗效。方法:选取2018年3月~2019年12月在我院放疗中心接受放射治疗的鼻咽癌放疗患者96例,采用硬币法随机分为对照组和观察组,每组各48例。对照组患者采用医用射线防护喷剂(健达泽)进行治疗,观察组患者采用复方大黄白及喷雾进行治疗,于放疗前后给药,直至放疗治疗结束。观察两组治疗前后临床疗效、中医症候积分、NRS疼痛评分、改良版皮炎SCORAD评分、皮肤病生活质量量表(DLQI)评分及血清炎性细胞因子[白介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、降钙素原(PCT)、表皮细胞生长因子(EGF)]、血常规、肝肾功能等的变化。结果:治疗后,观察组总有效率显著高于对照组(P<0.05);治疗后,两组中医证候评分明显低于治疗前(P<0.05),且观察组治疗后中医证候评分明显低于对照组(P<0.05);治疗后,两组NRS疼痛评分、改良版皮炎SCORAD评分、DLQI评分明显低于治疗前(P<0.05),且观察组治疗后各项评分明显低于对照组(P<0.05);治疗后,两组IL-6、hs-CRP、PCT水平明显低于治疗前(P<0.05),EGF水平明显高于治疗前(P<0.05),且观察组治疗后与对照组比较有显著差异(P<0.05);两组患者治疗前后血常规、肝肾功能等安全性指标均未见明显异常,均未出现皮疹等皮肤过敏不良反应。结论:自制复方大黄白及喷雾对于鼻咽癌放射治疗所致急性放射性皮炎患者临床症状的改善具有明显疗效,能够显著改善患者疼痛状况,改善皮肤情况,降低患者炎性水平,提高治疗效果,值得临床推介。
Objective:To investigate the clinical efficacy of applying Self-made Compound Dahuang Baiji Spray external treatment in preventing and treating acute radiodermatitis caused by radiotherapy of nasopharyngeal carcinoma.Methods:Ninety-six patients with nasopharyngeal carcinoma who received radiotherapy at the radiotherapy center of our hospital from March 2018 to December 2019 were randomly divided into a controlled group and an observation group by coin tossing,with 48 cases in each group.The patients in the controlled group were treated with protective spray against rays for medical use(Jiandaze),and the patients in the observation group were treated with Compound Dahuang Baiji Spray,and they were treated before and after radiotherapy until the end of radiotherapy.The clinical efficacy,TCM syndromes scores,NRS pain score,modified dermatitis SCORAD score,Dermatology Life Quality Index(DLQI)score,serum inflammatory cytokines[interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT),epidermal growth factor(EGF)],blood routine and liver and kidney function were observed in the two groups before and after treatment.Results:After treatment,the total effective rate in the observation group was significantly higher than that in the controlled group(P<0.05).After treatment,the scores of TCM syndromes in the two groups were significantly lower than those before treatment(P<0.05),and the scores of TCM syndromes in the observation group after treatment were significantly lower than those in control group(P<0.05).Aftertreatment,the NRS pain score,modified dermatitis SCORAD score,and DLQI score of the two groups were significantly lower than those before treatment(P<0.05),and the scores of items in observation group after treatment were significantly lower than those in the controlled group(P<0.05).After treatment,the levels of IL-6,hs-CRP and PCT in the two groups were significantly lower than those before treatment(P<0.05)while the level of EGF was significantly higher than that before treatment(P<0.05),and there were significant differences between the observation group and the controlled group after treatment(P<0.05).There were no obvious abnormalities in blood routine,liver and kidney function and other safety indicators between the two groups before and after treatment,and there were no skin allergic reactions such as rashes.Conclusion:Self-made Compound Dahuang Baiji Spray has obvious efficacy in the improvement of clinical symptoms of patients with acute radiodermatitis caused by radiotherapy of nasopharyngeal carcinoma.And it can significantly improve the pain status,improve the skin condition,reduce the inflammatory level,and enhance the treatment effect.Thus it is worthy of clinical recommendation.
