摘要
目的探讨高血压基底节区脑出血手术治疗中实时超声导航的应用情况。方法选择2020年1月-2021年3月本院收治的72例高血压基底节区脑出血行手术治疗的患者作为研究对象,随机分为2组,各36例。对照组采用显微镜下经侧裂入路进行基底节区血肿清除,观察组采用实时超声导航辅助显微镜下经侧裂入路进行基底节区血肿清除,比较两组手术情况、术后1d血肿清除率、采用日常生活能力量表ADL评价两组术前、术后半年疗效,并记录两组肺部感染、消化道出血、脑梗塞并发症发生情况。结果观察组手术、住院时间短于对照组,(P<0.05);两组术中出血量比较(P>0.05);观察组清除率>90%、清除率50%-90%均高于对照组(P<0.05);两组术后ADL评分均低于术前(P<0.05);观察组ADL评分均低于对照组(P<0.05);两组术后并发症比较(P>0.05)。结论在高血压基底节区脑出血手术治疗中采用实时超声导航,可促进患者康复,提高血肿清除率,改善患者日常生活能力,且并发症较小。
Objective To investigate the application of real-time ultrasound navigation in the surgical treatment of hypertensive basal ganglia hemorrhage.Methods From January 2020 to March 2021,72 patients with hypertensive basal ganglia intracerebral hemorrhage treated by surgery in our hospital were randomly divided into two groups,36 cases in each group.The control group used the microscope-assisted transfission approach to remove the basal ganglia hematoma,and the observation group used the real-time ultrasound navigation-assisted microscope to assist the transfission approach to remove the basal ganglia hematoma,The complications of pulmonary infection,gastrointestinal bleeding and cerebral infarction were recorded.Results The operation and hospital stay of the observation group were shorter than those of the control group(P<0.05);The intraoperative blood loss was compared between the two groups(P>0.05);The clearance rate>90%and 50%-90%in the observation group were higher than those in the control group(P<0.05);The ADL scores of the two groups after operation were lower than those before operation(P<0.05);The ADL score of the observation group was lower than that of the control group(P<0.05);The postoperative complications of the two groups were compared(P>0.05).Conclusion In the surgical treatment of hypertensive basal ganglia hemorrhage,the use of real-time ultrasound navigation can promote the rehabilitation of patients,improve the hematoma clearance rate,improve the ability of daily living of patients,and has less complications.
