Effects of Different Hemodialysis Modes on Brain Natriuretic Peptide and Left Ventricular Function in Patients with Uremia

摘要 目的探究不同血液透析模式对尿毒症患者血清脑钠肽(BNP)和左心功能的影响。方法选择2020年10月-2021年9月在茂名石化医院及茂名市人民医院行维持性血液透析治疗的81例慢性肾衰竭患者作为研究对象,根据血液透析模式不同分为3组,每组各27例。其中,A组采取血液透析、血液滤过及血液灌流(HD+HDF+HP)的方式;B组采取血液透析联合血液滤过(HD+HDF)的方式;C组采取血液透析(HD)的方式。比较三组治疗前后血清BNP、超声检测心功能指标变化情况。结果治疗前,三组血清BNP、超声指标(LVDd、LVDs、LVVs、LVPWT、IVST、E/A、LVEF)检测结果比较差异无统计学意义(P>0.05)。2个月后,A组血清BNP、超声检测LVDd、LVDs、LVVs、LVPWT、IVST值显著低于治疗前B组、C组,且LVEF明显高于B组、C组(P<0.05)。B组与C组上述各项检测指标结果差异有统计学意义(P<0.05)。三组E/A值比较差异无统计学意义(P>0.05)。结论尿毒症患者采取血液透析、血液滤过及血液灌流联合模式治疗能够有效减轻血清BNP水平,显著改善左心功能,这对于降低血透患者心血管疾病发生风险具有重要的意义。 Objective To explore the effects of different hemodialysis modes on serum brain natriuretic peptide(BNP)and left ventricular function in patients with uremia.Methods 81 patients with chronic renal failure who underwent maintenance hemodialysis in Maoming Petrochemical hospital and Maoming people’s Hospital from October 2020 to September 2021 were selected as the research objects.According to different hemodialysis modes,they were divided into 3 groups with 27 cases in each group.Group A was treated with hemodialysis,hemodiafiltration and hemoperfusion(HD+HDF+HP).Group B was treated with hemodialysis combined with hemodiafiltration(HD+HDF).Group C was treated with hemodialysis(HD).The changes of serum BNP and ultrasonic cardiac function indexes were compared among the three groups before and after treatment.ResultsBefore treatment,there were no significant difference in serum BNP and ultrasonic indexes(LVDD,LVDS,LVVs,LVPWT,LVST,E/A and LVEF)among the three groups(P>0.05).After 2 month,serum BNP,ultrasound LVDd,LVDs,LVVs,LVPWT and LVST values of group A were significantly lower than group B and C before treatment,and LVEF was significantly higher than group B and C,and there were statistically significant differences between group B and group C(P<0.05).There was no significant difference in E/A among the three groups(P>0.05).Conclusion The combined treatment of hemodialysis,hemodiafiltration and hemoperfusion in uremic patients can effectively reduce the level of serum BNP and significantly improve left ventricular function,which is of great significance to reduce the risk of cardiovascular disease in hemodialysis patients.
