摘要
目的探讨雷公藤多苷片联合硫酸羟氯喹治疗口腔扁平苔藓的疗效分析。方法选取医院收治的口腔扁平苔藓患者118例,采用随机对照组法将患者随机分为对照组(60例)和观察组(58例)。对照组给予硫酸羟氯喹,观察组在对照组基础上给予雷公藤多苷片,对比两组临床疗效、VAS评分和口腔糜烂面积、体征评分、生活质量评分、唾液相关因子及氧化应激指标水平。结果治疗后,观察组总有效率较高(P<0.05);治疗前两组VAS评分和口腔糜烂面积差异无统计学意义(P>0.05);治疗后两组VAS评分和口腔糜烂面积显著降低(P<0.05);并且观察组降低较明显(P<0.05);治疗前两组体征评分差异无统计学意义(P>0.05);治疗后两组体征评分明显降低(P<0.05);且观察组降低较明显(P<0.05);治疗前两组唾液免疫球蛋白A(IgA)、白蛋白(ALB)、干扰素-γ(IFN-γ)水平差异无统计学意义(P>0.05);治疗后两组唾液ALB水平明显降低,唾液lgA、IFN-γ水平明显升高(P<0.05);且观察组改善较明显(P<0.05);治疗前两组生活质量评分差异无统计学意义(P>0.05);治疗后两组精神健康、总体健康、情感职能、社会功能等评分明显升高(P<0.05);且观察组升高较明显(P<0.05);治疗前两组血清丙二醛(MDA)、脂质过氧化物(LPO)和超氧化物歧化酶(SOD)水平差异无统计学意义(P>0.05);治疗后两组血清MDA、LPO水平明显降低,血清SOD水平明显升高(P<0.05);且观察组改善较明显(P<0.05)。结论采用雷公藤多苷片联合硫酸羟氯喹治疗口腔扁平苔藓具有较好的临床疗效,可缓解疼痛度,减小口腔糜烂面积。
Objective To investigate the efficacy of Tripterygium Wilfordii Polyglycoside Tablet combined with hydroxychloroquine sulfate in the treatment of oral lichen planus.Methods A total of 118 patients with oral lichen planus admitted to the hospital were selected and divided into control group(60 cases)and observation group(58 cases)by the method of randomized control group randomly.The control group was given hydroxychloroquine sulfate and the observation group was given Tripterygium Wilfordii Polyglycoside Tablet on the basis of the control group.The clinical efficacy,VAS score,oral erosion area,physical sign score,quality of life score,salivary related factors and oxidative stress index levels were compared.Results After treatment,the total effective rate in the observation group was higher(P<0.05).There was no difference in VAS score or oral erosion area between the two groups before treatment(P>0.05).After treatment,VAS score and oral erosion area in the two groups were significantly decreased(P<0.05).And those of the observation group were decreased significantly(P<0.05).Before treatment,there was no difference in the scores of physical signs between the two groups(P>0.05).After treatment,the scores of the two groups were significantly decreased(P<0.05).And those of the observation group were significantly decreased(P<0.05).Before treatment,there were no significant differences in the levels of IgA,ALB or IFN-γin saliva between the two groups(P>0.05).After treatment,the salivary ALB level was significantly decreased,while the salivary IgA and IFN-γlevels were significantly increased(P<0.05).And the levels of the observation group were significantly improved(P<0.05).There was no difference in quality of life scores between the two groups before treatment(P>0.05).After treatment,the scores of mental health,general health,emotional function and social function in the two groups were significantly increased(P<0.05).And the those of the observation group were significantly increased(P<0.05).The levels of MDA,LPO and SOD in serum were not significantly different between the two groups before treatment(P>0.05).After treatment,the serum levels of MDA and LPO were significantly decreased,while the serum SOD level was significantly increased(P<0.05).The improvement in the observation group was significant(P<0.05).Conclusion Treating oral lichen planus with Tripterygium Wilfordii Polyglycoside Tablet combined with hydroxychloroquine sulfate has good clinical efficacy,which can relieve the degree of pain and reduce the area of oral erosion.
