摘要
目的研究达格列净联合常规药物治疗对心力衰竭(心衰)伴左室射血分数(LVEF)降低患者心功能及心血管不良事件的影响。方法将79例心衰伴LVEF降低患者纳入研究,按照随机数字表法分为观察组(40例,接受达格列净联合常规药物治疗)和对照组(39例,接受常规药物治疗)。对比两组心功能指标[左室舒张末期内径(LVEDd)、左室收缩末期内径(LVESd)、LVEF]水平,心血管不良事件发生率及明尼苏达心力衰竭生活质量量表(MLHFQ)评分。结果治疗后,观察组LVEDd、LVESd分别为(54.36±2.19)、(39.58±1.93)mm均小于对照组的(56.75±2.44)、(43.49±2.17)mm,LVEF为(45.64±1.25)%高于对照组的(44.85±1.32)%(P<0.05)。两组心血管不良事件发生率对比无差异(P>0.05)。治疗后,观察组MLHFQ评分(62.78±5.66)分低于对照组的(67.25±6.11)分(P<0.05)。结论在常规药物治疗基础上增用达格列净有助于改善心衰伴LVEF降低患者心功能,且不会大幅提升心血管不良事件的发生率,生活质量较优。
Objective To study the effect of dapagliflozin combined with conventional drugs on cardiac function and adverse cardiovascular events in patients with heart failure with reduced left ventricular ejection fraction(LVEF).Methods A total of 79 patients with heart failure with reduced LVEF were included in the study and divided into an observation group(40 cases,receiving dapagliflozin combined with conventional drugs)and a control group(39 cases,receiving conventional drugs)according to random numerical table.Comparison of cardiac function indexes[left ventricular end diastolic diameter(LVEDd),left ventricular end systolic diameter(LVESd),LVEF],incidence of adverse cardiovascular events and Minnesota Living with Heart Failure Questionnaire(MLHFQ)score between the two groups.Results After treatment,LVEDd and LVESd in the observation group were(54.36±2.19)and(39.58±1.93)mm,which were smaller than(56.75±2.44)and(43.49±2.17)mm in the control group;the observation group had a higher LVEF of(45.64±1.25)%than(44.85±1.32)%in the control group(P<0.05).There was no significant difference in the incidence of adverse cardiovascular events between the two groups(P>0.05).After treatment,the observation group had a lower MLHFQ score of(62.78±5.66)points than(67.25±6.11)points in the control group(P<0.05).Conclusion The addition of dapagliflozin to conventional drug therapy can improve the cardiac function in patients with heart failure with reduced LVEF without substantially increasing the incidence of adverse cardiovascular events,and the quality of life is better.
