摘要
骨是恶性肿瘤最常见的转移部位,骨转移通常会引发病理性骨折,顽固性疼痛以及运动障碍等一系列并发症。骨转移分为椎体转移与椎外转移,由于椎外骨转移较为少见,发生部位分散,针对不同的发生部位,在对患者的预后、生活质量、术后功能和术后并发症风险等进行综合考量后,需要选择不同的治疗方式,因此疼痛性椎外骨转移的治疗是姑息治疗中一个新的、隐蔽的、具有挑战性的领域。经皮骨成形术、肿瘤消融术、动脉化疗栓塞治疗、放射性粒子植入术等作为椎外骨转移治疗的常规微创手段,在骨肿瘤的局部治疗中发挥着越来越重要的作用,其中尤以经皮骨成形术为基础,彼此之间相互联合,可以产生不同的效应叠加,能够协同治疗、相互弥补,进一步提高椎外骨转移治疗的安全性与疗效,为椎外骨转移治疗提供了更佳选择。
Bone is the most common metastatic site of malignant tumors after lung and liver.Bone metastases often lead to complications such as pathological fractures,intractable pain,and movement disorders.Bone metastasis is divided into vertebral metastasis and extravertebral metastasis,because extravertebral bone metastasis is relatively rare,the occurrence site is scattered,for different occurrence sites,after comprehensive consideration of the patient's prognosis,quality of life,postoperative function and risk of postoperative complications,it is necessary to choose different treatment methods,so the treatment of painful extravertebral bone metastasis is a new,hidden and challenging field in palliative care.Percutaneous osteoplasty,tumor ablation,arterial chemoembolization,radioactive particle implantation,etc.,as conventional minimally invasive means of extravertebral bone metastasis treatment,play an increasingly important role in the local treatment of bone tumors,especially based on percutaneous osteoplasty,which can combine with each other,produce different effects superimposed,synergize treatment,make up for each other,further improve the safety and efficacy of extravertebral bone metastasis treatment,and provide a better choice for extravertebral bone metastasis treatment.
