摘要
腰椎椎体后缘软骨板离断常被视为发生在生长发育期青少年的一种特殊类型的损伤。近年报道成年人例数的增加使以往在病因、病理、损伤机制等方面的认识有所改变。作者近来收治此类型损伤２１例，均为成年人。经Ｘ线片、脊髓造影和ＣＴ扫描的观察与分析后认为：青春期椎体软骨板的先天性脆弱和Ｓｃｈｍｏｒｌ结节的形成可能是成年后椎体后缘软骨板发生离断性改变的基础，而这种改变与急性创伤间无明显内在联系。本文就发病年龄、损伤机制、病理因素、间盘病损、手术治疗及损伤命名等问题进行了讨论。
In the past,avulsion of the lumbar posterior end plate was considered to be a special type of lesion in children and adolescents.We have now a better understanding of its causes,pathology and mechanism since adult cases kept increasing in recent years.The 21 patients treated in the past 3 years and 8 months were all adults.It is considered that the avulsion of lumbar posterior end plate in adults is caused by congenital fragility and formation of Schmorl′s node of lumbar vertebral posterior end plate in adolescence on basis of examinations by radiology,myelography and CT.The lesion is different from that resulted from acute trauma.Its onset age,pathogenesis,pathology,disc lesion,operative treatment and definition were discussed.
